Breast asymmetry — where one breast is noticeably larger, higher, or differently shaped than the other — is common. Most sources estimate that a majority of people have at least a slight difference between breasts, and a meaningful proportion have differences of a half-cup size or more.

The standard fitting approach handles most cases well, and there is a range of specific garment features that address the practical challenges of asymmetry without requiring custom-made pieces.

The standard approach: fit the larger side

The foundational fitting principle for asymmetrical breasts is to size the bra to the larger breast. The smaller breast side will have some excess cup volume; this is managed with padding.

At the measurement stage, take both measurements and use the larger. If the right breast is a D cup and the left is a C cup, buy a D cup and address the smaller side.

Padding inserts for the smaller cup

Most quality bra cups can accommodate a removable padding insert in the smaller cup. The insert fills the excess volume, provides visual symmetry, and prevents the cup from gaping or moving during wear.

Types of inserts:

  • Foam inserts ("cookies"): half-circle or full-circle foam pads placed inside the cup, typically from the bottom. Lightweight, washable, available in many sizes.
  • Silicone inserts: heavier, more realistic in feel and appearance under sheer fabric, and better suited to larger volume differences.
  • Fiberfill pouches: soft, lightweight, very low-profile under clothing.

Many quality bras have a small pocket at the base of each cup designed to hold inserts. If yours does not, a light foam insert behind the cup is often sufficient.

Garment features that accommodate well

Moulded foam cups: hold their three-dimensional shape regardless of whether the cup is fully filled, which prevents the collapsing or gaping on the smaller side that unstructured cups can show.

Stretch lace: the natural stretch of lace accommodates minor volume differences without obvious visual asymmetry.

Independent adjustable straps: allow the strap on the higher or smaller breast to be shortened, raising the cup on that side to match the position of the cup on the larger side. This is a meaningful comfort and appearance improvement for people where the asymmetry includes a height difference.

Convertible bras: allow strap position adjustment that can help with asymmetric shoulder or breast height.
